Rapid Transition Housing Program
*Please note, this program is not emergency housing and the process is not immediate.*
RTHP provides a rental subsidy and supportive housing services for high-need Medicaid beneficiaries. The program is a Medicaid Redesign Team initiative funded by the Department of Health and administered by the New York Association on Independent Living, and is available in four areas of the state: Long Island, Syracuse, New York City, and Rochester.
Housing Specialists are staffed to assist eligible individuals in locating and securing safe and affordable housing within program guidelines. Individuals sign their own leases and arrange for utilities to be setup under their name.
Once determined eligible for the program, they are then assigned to an Independent Living Specialist to devise individualized service plans and assist in linking the participant to community resources for maintaining independence.
The RTHP subsidy is modeled after the Housing Choice Voucher (Section 8) and Olmstead Housing Subsidy program in that
the individual is required to pay 30% of their monthly income,
less allowances, as their portion of rent so long as the unit remains within the Fair Market Rent guidelines.
There is additional funding available for community transition services which includes security deposits, moving expenses, and the purchase of household necessities.
Program Eligibility

Homeless or unstably housed
-
Active Medicaid
-
Proof of homelessness and ability to live safely and independently in the community

1 or more documented chronic physical disabiility OR 1 chronic condition
Or have at least one of the following:

2 or more inpatient stays in the last 12 months

5 or more emergency department visits in the last 12 months

4 or more emergency department visits and 1 or more inpatient stay in the last 12 months
-
1 skilled nursing/rehab facility stay of 30 days or longer in the past 12 months
